ARA-290 (Cibinetide) is a synthetic 11-amino acid cyclic peptide derived from the helical region of erythropoietin (EPO). Unlike full-length EPO, ARA-290 does not bind to the classical EPO receptor (EPOR homodimer) responsible for erythropoiesis, but instead selectively targets the innate repair receptor – a heterodimer of EPOR and the common beta receptor (?cR). This selectivity allows researchers to study EPO’s tissue-protective functions in isolation from its hematopoietic effects.

Researchers have studied ARA-290 for its potential to activate innate repair receptor signaling pathways involved in tissue protection and anti-inflammatory responses. Preclinical studies have examined its effects in models of peripheral neuropathy, where research has explored its influence on small fiber nerve repair and regeneration. Studies have also investigated its immunomodulatory properties, including effects on macrophage polarization toward anti-inflammatory phenotypes.

Research programs have examined ARA-290 in models of metabolic dysfunction, including studies in rodent models of induced diabetes exploring its effects on pancreatic islet preservation and inflammatory cytokine profiles. Clinical research has generated data on its effects in sarcoidosis-related small fiber neuropathy and other inflammatory conditions. Its selective tissue-protective mechanism makes it a valuable research tool for studying EPO biology beyond red blood cell production.
